Report of Death/Consular Mortuary Certificate

Please see below list of requirements for the filing of Report and Death and issuance of Consular Mortuary Certificate: (For non-Filipino citizens, only the Consular Mortuary Certificate is required if to be shipped to the Philippines)

Note: It is important to send us the scanned copies of the documents through email for pre-evaluation before sending the hard copies via mail OR before having the forms notarized (if the informant cannot come personally to the Embassy to submit and sign on the documents).
Please note we do not accept Report of Death forms that are acknowledged by the Justice of the Peace (JP).

Requirements for Report of Death:

  1. Consular Service Slip - 1
  2. Report of Death Form (ROD form) - 4 originals (All forms must be typed and printed in BLACK INK)
  3. Death Certificate - original and 4 copies
  4. Medical certificate - original and 4 copies
  5. Certificate of No Infectious Disease – 4 copies (not required for shipment of urn)
  6. If cremated, Certificate of Cremation – 4 copies
  7. Passport – 4 copies
  8. Visa copy of the deceased
  9. Courier Bag with a “Courier Signature Required” sticker (within NZ if residing in NZ/ International courier if residing outside NZ)

Requirements for Report of Fetal Death

The Philippine Statistics Authority (PSA) defines Fetal Death as the death prior to the complete expulsion or extraction from its mother of a product of conception, irrespective of the duration of pregnancy.

  1. Consular Service Slip - 1 original
  2. Report of Fetal Death Form (ROFD form) - 4 originals (All forms must be typed and printed in BLACK INK)
  3. Death Certificate - original and 4 copies
  4. Medical certificate, if applicable - original and 4 copies
  5. Certificate of No Infectious Disease – 4 copies (not required for shipment of urn)
  6. If cremated, Certificate of Cremation – 4 copies
  7. Passport of parents - 4 copies
  8. Marriage certificate of parents, if married - 4 copies
  9. Courier Bag with a “Courier Signature Required” sticker (within NZ if residing in NZ/International courier if residing outside NZ)

Requirements for Consular Mortuary Certificate: (Repatriation of Remains)

To transport human remains of a Filipino citizen to the Philippines in a coffin or an urn, one must apply for a Consular Mortuary Certificate at the Philippine Embassy.

  1. Death Certificate - original and 1 copy
  2. Medical certificate - original and 1 copy
  3. Certificate of No Infectious Disease – 1 copy (not required for shipment of urn)
  4. If cremated, Certificate of Cremation – 1 copy
  5. Passport – 1 copy
  6. Flight itinerary of the remains, including the name address and telephone number of the receiving funeral home or consignee in the Philippines; address and phone number of the next of kin in New Zealand and in the Philippines;
  7. Name, Photocopy of the identification page of the person accompanying the remains
  8. Courier Bag with a “Courier Signature Required” sticker (within NZ if residing in NZ/ International courier if residing outside NZ)

The Consular Officer reserves the right to require additional proof and/or documents from the Informant (ex. Birth Certificate, Marriage Certificate, etc.) to determine his/her citizenship, identity, relationship with the surviving spouse or relative or eligibility for registration of death under Philippine laws.
